The club was born on 5 March 1904 in the city of Buenos Aires under the name San Telmo Football Club, on the initiative of Pantarotto, its first president. Its first shirt was blue and white, but according to club tradition the blue ran into the white, and so the colours became blue and sky blue. Its first ground of its own was at Azopardo and Garay, and on 23 May 1923 it adopted its definitive name, Club Atlético San Telmo.

It is San Telmo's ground on Isla Maciel, at Las Heras and Vieytes, just metres from the Riachuelo. The club arrived there after being evicted in 1926 from its Azopardo and Garay ground and played its first official match there in November 1929, against Alvear. Since 1983 it has been named after Dr. Osvaldo Francisco Baletto. It was closed between February 2006 and October 2011, following incidents against Talleres de Remedios de Escalada, and in 2015 the new concrete Las Heras stand was opened, raising its capacity to around 10,000.

The fans
Its fans, like the team, are known as “candomberos”. The big derby is against Dock Sud, its neighbour in the area: the first meeting was in 1928, and the first of the professional era, on 22 June 1957, ended 4-1 to San Telmo. It also has an old neighbourhood rivalry with Barracas Central, dating from the days when the club was based in San Telmo, with a first match in 1919. The club celebrated Fans' Day for the first time on 20 December 2007, the same day of the year on which it had won promotion to the Primera División in 1975.
